Issuance of Initial License to Operate / Authority to Operate of a Regulated Health Facility
Receipt of DOH-Initial application/s starts from the 1st working day of the year until November 15 of the same year (as per AO No. 2019-0004), available Monday-Friday, 8am to 5pm excluding holidays and work suspensions.

Receipt of Renewal application/s starts from October 1 until December 15 of the same year (as per AO No. 2019-0004), available Monday-Friday, 8am to 5pm excluding holidays and work suspensions. Health facilities can still renew their LTO/COA/ATO after the cut-off date (December 15) on January 1 to March 31 but with 100% surcharge and gap in the validity of the authorization (as per AO No. 2019-0004)

1. What is RLED Information Management System?
RLED Information Management System is used to facilitate the collection, storage, organization, tracking and monitoring of information of Health Facilities in Region 6. RLED Information Management System was developed in line with the 8-Point Action Agenda which is digitalization that aims for efficient, faster and accessible health service delivery.
2. How to login on RLED-IMS?
Curretly, RLED-IMS login page will only be accessible to RLED Personnel
3. What will I do if I forgot my password?
In case you forgot your password, users may contact the System Administrator for account recovery.
4. Can the RLED-IMS be viewed even without an account?
Yes. The RLED-IMS can be viewed though there is no account, In Public View, user can access the issuances, licensing forms and other downloadable forms. However, only those with an existing account can see specific information such as details of health facilities in their areas.
5. Who can add and update existing information of Licensed health facilities?
Focal Persons with accounts can add, edit, and delete information on health facilities. delete and Update buttons can be found at the leftmost side of the webpage together with the view button while add button can be seen on the side navigation bar under Services.
6. How can I download data from the system?
Users can download data by clicking on the button located at the rightmost side of the table. Data can be exported to pdf, excel and csv format.
